An ear infection (a real one--they can be misdiagnosed or over diagnosed) will always have/be associated with fluid build up in the ears. Most commonly that means that your child will have been congested (nasal passages or head cold type stuff). Generally there's a fever--but not always. Ear infections will also always feel 'worse' at night.
Like the others have said, I would make an appointment to get it checked out. However even if it is, antibiotics are not always the way to go in children under 2. Try pushing fluids. . .and lots of them to help encourage his body to heal. You can also give Tylenol or Motrin at night to help ease the pain and/or homeopathic ear drops to help ease the pain. Also, things like hanging out in a hot muggy steamy bathroom are very helpful.
