Hi Barbara,
I did this rebate last year and received a rebate check for it without a problem. I just printed the form that I had found on that same website for drugstore.com and submitted it with the required items. I wasn't sure I'd get the rebate since I didn't order from the website - I bought the toothbrush last year when CVS had the ECBs for it - so I made money when I got the rebate check in the mail!
Try it and hopefully you'll receive your rebate check too!
