Default scrubbing bubbles shower cleaner MIR?

I see in the database that there is a ($10, I think?) Mail in rebate for the scrubbing bubbles automatic shower cleaner. Do I need a form for that and does anyone know where I can get one???

Yes, I used the coupon. I was also hoping for the rebate to go with it.

Rite Aid has them this week for 18.99 which is 8.99 with the coupon and they have a $5 Single Check Rebate on them which makes it 3.99.

Don't know if this helps
