Default spit up question

My sis in law asked me today what could be causing her 3 week old to be spitting up so much. I told her what i knew: try a different bottle, switch formula, sit him up after feedings for about 20 minutes. She is currently feeding him both formula and breast feeding. Formula when they are out of the house and BFing when at home.
Any other suggestions to help with spit up?

What is he spitting up? The breastmilk or the formula or both? Does he scream when she tries to lay him down?

One of mine just spit up a lot. She has trouble eating food. . .like different textures bother her. . .bc she has an overactive gag/reverse parastalsis reflex but there wasn't anything to be done except carry around extra spit up cloths! (This is rare though from what I understand.)

He is spitting up both breast milk and formula. She is just frustrated when he does it after nursing he seems hungry again almost instantly. So she is bfing ALOT! I know that nursing alot is all well and fine, but she says its ruining her day, she can't do ANYTHING and her 2 year old is not making things any easier....she is at her wits end and now with the spitting up she is just about ready to quit bfing all together just so she has more time. She is a great mom just very frustrated right now.

You may (if it seems appropriate) want to point out to her that if he's spitting up formula too. . .switching will not increase her time right now. But my next thought would be is she currently eating any diary products? Since you're online we can try a normal-ish conversation?!

She should stop immediately and stop giving him formula and see if that helps. No butter, no ice cream, no cheese, nothing with dairy in it.
Also, you said he seems hungry all the time. Does this sound right? She nurses him, he's half asleep, she attempts to put him down and he cries? Or no?
The two most common/logical explanations would be a sensitivity to dairy and reflux.

Oh. . .edit that to say that if she feels like she has to top off with formula then she should try soy formula. . .no regular formula or dairy for her for 48 hours and see if that helps.

That is exactly what he does. He falls asleep and doesn't "finish" then tries to put him down and he wakes crying for more milk. She has him on soy now, just happened to have that as a sample can.
I am going to call her tomorrow morning and tell her to stop eating dairy and see if that helps.

Thats also when she ends up giving him some formula to (like you said) "top him off" I feel sorry for her, she feels like she just isn't giving him what he needs and she is so tired and frustrated.
